目的：对比两种不同缝合方式在后腹腔镜肾部分切除术(LPN)中的疗效。方法99例肾细胞癌(RCC)患者采用LPN治疗,随机分为两组。V-loc单向倒刺可吸收线组42例(采用2-0 V-loc单向倒刺可吸收线连续缝合技术), Vicryl可吸收线组57例(采用2-0 Vicryl可吸收线联合Hem-o-lok的无结连续缝合技术)。对两种术式手术时间、热缺血时间、术中平均出血量、术后住院时间、术后并发症发生率等进行统计分析。结果99例患者手术均获成功,无中转开放。两组手术时间分别为(94.0±29.0)min和(109.0±26.0)min,热缺血时间分别为(13.0±2.6)min和(21.0±3.4)min,术中出血量分别为(82.0±20.0)ml和(118.0±22.0)ml,组间比较差异均有统计学意义(P<0.05)。所有患者手术切缘均为阴性。结论 V-loc单向倒刺可吸收线在后腹腔镜肾部分切除术中应用能够明显缩短缝合时间和手术时间,并未导致手术并发症增加,具有良好的安全性。%Objective To compare the curative effects between two different suture methods in retroperitoneal laparoscopic partial nephrectomy (LPN).Methods A total of 99 patients with renal cell carcinoma (RCC) treated by LPN were randomly divided into two groups, as V-loc one-way barbs absorbable suture group with 42 cases (2-0 V-loc one-way barbs absorbable suture technique) and Vicryl absorbable suture group with 57 cases (2-0 Vicryl absorbable suture combined with Hem-o-lok in non-knot continuous suture technique). A statistical analysis was made on the operation time, warm ischemia time, average intraoperative bleeding volume, postoperative hospital stay, and incidence of postoperative complications of the two groups.Results All the 99 cases underwent successful operation, and none of them received alternative open operation. The two groups had operation time as (94.0±29.0) min and (109.0±26.0) min, warm ischemia time as (13.0±2.6) min and (21.0±3.4) min, intraoperative bleeding volume as (82.0±20.0) ml and (118.0±22.0) ml. The differences between the two groups all had statistical significance (P<0.05). All patients had negative surgical margin.Conclusion Implement of V-loc one-way barbs absorbable suture can remarkably shorten suture time and operation time in retroperitoneal laparoscopic partial nephrectomy. This method does not increase complications, and it contains good safety.
