Survivin基因在膀胱癌中的表达及其与Bcl-2 和Caspase-3基因表达的相关性

[ Objective] To study the expression of Survivin gene and its relationship with expression of Bcl-2 and Caspase-3 in bladder transitional cell carcinoma (BTCC). [Methods] Immunohistochemistry(SP) was performed to detect the expression of Survivin, Bcl-2 and Caspase-3 in 40 cases of BTCC. [Results] Among 40 cases of BTCC patients, the rate of expression of Survivin was 67.5 %, and the expression-rate of Bcl-2 and Caspase-3 was 52.5 % and 45 % respectively. The positive expression of survivin was closely conelated with malignant degree,pathological grade, the state of onset (whether was relapse or not), and the positive expression of Bcl-2 and Caspase-3 (P ＜0.05), but not with age, sex and clinical stage (P ＞0.05). [Conclusions] The levels of survivin are elevated in BTCC. Survivin plays an important role in BTCC grade, metastasis or prognosis. The expression of Survivin restrains the activity of Caspase-3; Survivin and Bcl-2 act synergism in regulation of apoptosis of tumor. The expression of Survivin and Bcl-2 simultaneously in bladder carcinoma indicates the malignant degree is higher and the possibility of metastasis or relapse was stronger. Therefore, the process of state of disease should be closely observed.%目的探讨Survivin基因在膀胱移行细胞癌中的表达及其与Bcl-2和Caspase-3表达的相关性.方法应用免疫组化(SP法)检测40例膀胱癌组织中Survivin、Bcl-2和Caspase-3基因的表达.结果在40例膀胱癌患者中,Survivin基因的阳性表达率为67.5%,Bcl-2和Caspase-3的阳性表达率分别为52.5%和45%.Survivin的阳性表达与膀胱癌的恶性程度、病理分级、是否复发、Bcl-2和Caspase-3蛋白的阳性表达密切相关(P＜0.05),而与年龄、性别、肿瘤的临床分期无明显相关性.结论 Survivin基因在膀胱癌的恶性程度、肿瘤转移及预后等方面有较大的意义.Survivin基因表达抑制了Caspase-3的活性;Survivin与Bcl-2可能在调控肿瘤凋亡中的起协同作用,而若在膀胱癌中检测到Survivin和Bcl-2的表达,预示肿瘤的恶性程度较高,术后转移和复发的可能性较大,应密切观察病情进展.
