王家岭3·28透水事故获救矿工消化功能的分析

         

摘要

Objective To investigate the influence of hunger and stress on digestion. Methods 115 of the 153 miners, all males, aged (38.7±7.6)(19~ 54), who were trapped underground in the pit during the water inrush accident in Wangjialing Coal Mine on 28 March 2010 and suffered hunger and stress for 8 days, were rescued and hospitalized. Symptoms of digestive system were observed. Peripheral blood samples were collected for 7 ~ 10 days to examine the levels of aminotransferases, bilirubins, amylase, lipase, and electrolytes. Special diet regimens were developed. Results Twenty-one of the 115 miners developed abdominal discomfort or pain,and 2 developed nausea. 49.6% of the 115 cases showed hypoalbuminia, 33.3% of the cases showed increase of AST and/or ALT, 40.3% showed increase of total bilirubin, and 74.5% showed abnormality of electrolytes,including hyponatremia, hypokalemia and or hypocalcemia. Abstinence from food and water was enforced beginning from hospitalization. Two days later, liquid food was given, and then semi-liquid food, and at last starchy food was given first, and then protein and fat were added gradually. Conclusion Long-term hunger and stress cause malnutrition, hypoalbuminia, and disorders of blood biochemical indexes. Proper diet regimens and fluid therapy help recover.%目的 分析长期饥饿应激条件对消化系统的影响.方法 2010年3月28日王家岭煤矿透水事故中153名矿工,被困井下,处于断食、应激状态.8d后115例矿工获救并住院,均男性,年龄(38.7±7.6)岁(19~54岁).观察其消化系统症状,入院后连续7~10d检测转氨酶、胆红素、脂肪酶、淀粉酶、电解质等生化指标.结果 115例矿工中,21例矿工表现腹部不适和腹痛,有2例矿工表现恶心,但未有呕吐等症状;49.6% 的矿工有低白蛋白血症;33.3%的矿工丙氨酸氨基转移酶(ALT)和天冬氨酸氨基转移酶(AST)增高;40.3% 的矿工总胆红素增高;74.5%的矿工有电解质异常,表现为低钠血症、低钾血症和/或低钙血症.结论 长时间的饥饿应激可致低白蛋白血症及血生化紊乱,循序渐进的进食方案及补液治疗有助于恢复.
