胃切除大鼠小肠黏膜上皮内和固有层T淋巴细胞表达研究

     

摘要

Objective To observe the effects of stomach excision on Intraepithelial lymphocytes( IELs )and lamina propria lymphocytes ( LPLs )in small intestinal mucosa of rats. Methods Forty rats were averagely randomized into sham operation control group, model group with stomach excision. The rats in the sham operation group underwent an operation of abdominal midline incision and suture with the stomach retaining. After the operation, the rats were allowed to have a free access to water and foods. The rats in the model group under went an operation of stomach excision, then were given nutrition, an intacted protein enteral nutrition powder. One week after the operation , the rats were executed, and their intraepithelial lymphocytes and lamina propria lymphocytes in small intestinal mucosa were isolated. The T-cells subsets including αβT cell antigen receptor( αβTTCR )-cluster of differentiation 3 positive( CD3 + ), cluster of differentiation 4 positive( CD4 + )and cluster of differentiation 8 positive( CD8 + )were detected by using flow cytometry. Results The proportions of αβTTCR-CD3 + ,CD4+ ,CD8 + T of IELs in the model group decreased in different degree compared with those in the sham operation group, P < 0. 01 or 0.05, while the proportion of CD4 + T showed no significant changes in both group. The proportions of αβTTCR-CD3 + , CD4+ ,CD8 + T of LPLs in the model group decreased in different degree compared with those in the sham operation group,P <0. 01. Conclusion T cell of IELs and LPLs significantly reduced after the stress of stomach excision, which may lead to functional changes of intestinal mucosal immune barrier.%目的 探讨胃切除对大鼠小肠黏膜上皮内T淋巴细胞(Intraepithelial Lymphocytes,IELs)和固有层T淋巴细胞(Lamina Propria Lymphocytes,LPLs)的影响.方法 40只大鼠随机分为假手术对照组、胃切除模型组,每组20只.对照组大鼠仅给予腹部正中切开后缝合,不行胃切除,手术后自由饮水进食;模型组大鼠行胃切除手术,并予肠内营养制剂能全素作为基础能量供应.手术1周后,分离出两组大鼠小肠黏膜IELs和LPLs,采用流式细胞仪分别检测IELs和LPLs的αβTCR-CD3+、CD4+、CD8+T淋巴细胞.结果 (1)模型组IELs的αβTCR-CD3+、CD4+、CD8+T淋巴细胞构成比与对照组比较,均有不同程度的明显降低,除CD4+T降低无统计学意义外,余均P<0.01或0.05;(2)模型组LPLs的αβTCR-CD3+、CD4+、CD8+T淋巴细胞构成比与对照组比较,明显降低,差异有显著性,P<0.01.结论 胃切除手术创伤,可以导致大鼠IELs和LPLs的T淋巴细胞降低,引起肠黏膜免疫屏障障碍功能障碍.
