Rethinking Fidelity, Cognition and Strategy: Medical Simulation as Gaming Narratives

Simulations can transcend the literal representation of practice worlds. This paper considers the use of gaming and narrative to identify key underlying features and the ways in which they can be used in creating simulation activities. Fidelity has been defined as: "the accuracy of the representation when compared to the real world" [1]. The pursuit of fidelity and veracity in simulation has been a major part of the development of medical simulations (although interestingly a recent systematic review failed to identify fidelity as an essential factor in their efficacy [2]). Although fidelity is a major concern for simulation developers and practitioners there is relatively little consideration of what it is that is being made more real or true and why. A key question remains whether it is the fidelity of presentation or other factors that are critical to success. This paper considers fidelity factors other than presentation in different modalities of medical simulation.
