METHOD OF PREVENTING PURULENT COMPLICATIONS IN CASE OF ISOLATED RESECTION OF PANCREAS HEAD

FIELD: medicine.;SUBSTANCE: invention relates to medicine and can be applied for prevention of purulent complications in case of isolated resection of pancreas head. Main pancreatic duct of distal stump of pancreas is drained with silicone drainage. Modified parenchyma of pancreas head is maximally ablated with preservation of its separate parts, preserved are anterior and posterior upper pancreatic-duodenal arteries, intrapancreatic part of common bile duct. Omentum is lifted from abdominal cavity, placing it perpendicularly to patient's body axis, and intraoperative diaphanoscopy is performed with application of arc xenon lamp as illumination source. Flap of greater omentum is prepared with preservation of gastro-omental artery. Free edges of right half of omental flap are fixed by single interrupted sutures of monolithic thread on perimeter to remaining parenchyma of pancreas head and base of omental flap. Free edge of left part of omental flap is wrapped around pancreatoenteroanastomosis. Edges of flap are sewn to each other above pancreatoenteroanastomosis. Rubber dischargers are introduced into cavity of newly created formations made from flap of greater omentum - "omental bursas".;EFFECT: method makes it possible to reduce risk of purulent complications.;1 ex, 6 dwg
