Značaj preoperativnih vrijednosti koncentracije spermija i ukupnog broja progresivno pokretnih spermija u procjeni uspješnosti varikocelektomije

This study analyzed data obtained from 99 patients who had an open retroperitoneal varicocelectomy at the Department of Urology, General Hospital ʺHoly Spiritʺ over a period from 2001 until 2008. On all patients preoperatively and within 6 months after surgery a seminal analysis was done. We observed absolute and relative changes in the sperm concentration, the total number of progressively moving spermatozoa, the concentration of progressively moving spermatozoa, the percentage of progressively moving spermatozoa, and the total number of sperm in the semen. The goals of this study were to investigate whether patients who preoperatively have a lower sperm concentration of 10 x 106/ml and / or a total number of progressively moving spermatozoa is less than 5 x106 have significant postoperative recovery of seminal parameters. Another goal was to assess the impact of an open retroperitoneal varicocelectomy on the seminal parameters, and also to evaluate the impact of the age in which the varicocelectomy was performed to recovery in the investigated seminal parameters. The result of this study showed the justification of the varicocelectomy in patients who have sperm concentrations of less than 10x106/ml and / or a total number of progressively moving sperm count of less than 5X106, because in these patients we found significant improvement in the seminal parameters. Analyzing the overall results of all subjects we did not record a statistically significant difference between the preoperative and postoperative seminal parameters. When we extracted those patients who had a postoperative recovery in the seminal parameters we recorded a score of 52% successful varicocelectomy. Our research showed that the age of the operated patients did not influence the postoperative recovery of the seminal parameters.
