The literature inclusion criteria were: had to include empirical data analysis,had to be related to Medicaid-eligible children or comparisons of Medicaid-eligible children to other groups of children in the US; and the data had to reflect the economic, social and medical environment of the late 1980s and early 1990s, or earlier periods of the study is considered seminal or highly generalizable. Automated database searches included EPSDT crossed with child, Medicaid, evaluation studies, outcome and process assessment, delivery of health care standards, mass screening, national health programs, school health services, physician supply and distribution, physician practice patterns, referral and consultation, health services accessibility, preventive health services, comprehensive health services, immunizations, health status, lead and lead poisoning, vision screening and vision tests, dental prophylaxis and dental care, and hearing tests. In addition to automated searches, contents of current relevant health services and medical journals, conference programs, published bibliographies, current research reviews in health services newsletters and reference lists of relevant studies.
