Assistive Technology and Home Modification: Creating Systems Change for Older and211 Younger People with Disabilities

The initiative was designed to create systems change in how assistive technology211u001eand home accessibility services change in how assistive technology and home 211u001eaccessibility services are delivered. The initiative had three major objectives: 211u001eTo identify and promote the replication of models for systems change in assistive 211u001etechnology and home modification services; To advance a national agenda on the 211u001erole of assistive technology and home modification in serving the needs of older 211u001epersons and persons with disabilities by bringing together researchers, 211u001epractitioners, advocates and policy makers from the aging, disability and 211u001erehabilitation networks to share theoretical understandings, research findings 211u001eand tested practice approaches; and To develop and implement a new model of 211u001eservice coordination in the delivery of assistive technology and home 211u001emodification services in a multi-county region of Pennsylvania. These objectives 211u001ewere intended to increase national awareness of proven, as well as new, models 211u001efor the coordination of aging and disability systems related to assistive 211u001etechnology and home accessibility, and support their replication and 211u001edissemination.
