Service coordination has been identified as 'the key, the linchpin, the 211u001eglue of successful programs to link housing and services.' This guide considers 211u001eservice coordinators' roles, the problems they face, and strategies that they, as 211u001ewell as aging network and other professionals can use to address these problems.
