Building a Community Assisted Independent Living System. Final Report. Title IV211 Grant

The Vermont Department of Aging and Disabilities (DA&D) used a grassroots211u001eplanning model to determine what consumers, providers and advocates needed and 211u001ewanted in a community-based long term care service delivery system, which would 211u001eserve both elders and younger adults with disabilities. The infrastructure of a 211u001enew Community-Assisted Independent Living System (CAILS) would focus on the 211u001edevelopment of a single point of entry for both older persons and younger adults 211u001ewith disabilities. The Area Agencies on Aging were seen as the focal point of 211u001ethis coordinated and consolidated system. Key elements of the infrastructure were 211u001edeveloped (uniform assessment tool, comprehensive database, information and 211u001eassistance system and case management system), however, implementation of the 211u001emain component of the system, a single entry point for consumers, remains 211u001eelusive.
