Health Education and Social Service/Primary Care Coordination Demonstration Project for a Rural Area - Outreach Project

The existing 'state of the art' concerning the availability and need for health and human services within the Region and the real perceived needs of the elderly persons residing in the service area were studied. Health education programs were offered at thirty-six sites, and health screening tests were offered at fifty sites. Sites include senior centers, diner's clubs, Older American Social Centers, churches, hi-rise apartments, elderly housing complexes and other facilities. Transporation was provided to most of the sites at no expense to thhe individual. A total of 1,693 unduplicated individuals participated in the health screening services, over 350 of whom participated in a second health screening and 70 of whom participated in three health screenings. Health education programs were viewed by 9,808 participaants.
