A 73-year-old man with recurrent respiratory infections was seen as an outpatient after a hospitalization for an acute pneumonia. He had a history of ischemic heart disease (ejection fraction of 20%) and chronic colitis of indeterminate cause. Figure 1 shows a chest radiograph obtained at the time of admission. Figure 2 shows axial and coronal images from a subsequent computed tomographic (CT) examination of the chest.
