Extensor pollicis longus tendon rupture with concomitant rupture of the extensor digitorum communis II tendon after distal radius fracture

Dear Sir, A 60-year-old man was referred to our clinic with a rupture of his extensor pollicis longus (EPL) tendon after tendon and EDC III tendon (Sadr, 1984), but this did not affect the reconstruction of the EPL tendon. In contrast, rupture of fourth compartment extensor tendons can occur after palmar plating of distal radius fractures as a result of impingement on prominent screw tips (Al-Rashid et al., 2006; Hattori et al., 2008). The EIP transfer is the most common procedure for EPL rupture, but can only be used if EDC II is intact. In the event that EDC II is also ruptured, the cause should be sought in case other tendons are at risk. Alternative transfers to EPL include extensor pollicis brevis, extensor carpi radialis and a slip of abductor pollicis longus (Gelb, 1995). We recommend that the integrity of EDC II is checked at wrist level before transecting the EIP tendon.
