An 88-year-old man, with no known history of structural heart disease, was admitted to the emergency department complaining of regular palpitations. There was no history of presyncopal or syncopal episodes and the physical examination was unremarkable. The patient was not on any antiarrhythmic medication and serum ion levels were normal. Baseline electrocardiogram (EKG) revealed a wide QRS complex tachycardia of 128 beats per minute (bpm) with left bundle branch block (LBBB) and left superior axis deviation morphology with no clearly apparent atrial activity (Fig. 1).
