Objective To investigate the expression of NOB- 1 and CyclinD1 in papil ary thyroid carcinoma and its signifi-cance. Methods Immunohistochemistry was used to detect the expression of NOB- 1 and CyclinD1 in 68 cases of thyroid pap-il ary carcinoma, 30 cases of nodular goiter and 18 cases of normal thyroid tissue. Results In normal thyroid tissue, nodular goi-ter, thyroid papil ary carcinoma tissues the positive rates of NOB- 1 expression were 6.67%, 13.33%and 77.94%respectively(P<0.01);those of CyclinD1 expression were 0.00%, 20.00%, 83.82%, respectively (P<0.01). NOB- 1 positive rates in thyroid papil-lary carcinoma stageⅢ,Ⅳwere higher than those in stageⅠ,Ⅱ(91.43%vs 63.64%, P<0.05);those with lymph node metasta-sis were higher than those without lymph node metastasis (96.00% vs 67.44%, P<0.05); those with capsule involvement were higher than those without capsule invasion (95.65% vs 40.91%, P<0.01). Positive rates of CyclinD1 in stage Ⅲ, Ⅳ carcinoma were higher than those in stageⅠ,Ⅱ (94.29%vs 72.73%, P<0.05);those with lymph node metastasis were higher than those without lymph node metastasis (100.00%vs 58.88%, P<0.01);those with capsule invasion were higher than those without cap-sule invasion (97.83%vs 54.55%, P<0.01). With the progress of the disease, the positive rate of NOB- 1 and CyclinD1 were in-creased, and NOB- 1 and CyclinD1 were positively correlated with each other (r=0.344, P<0.01). Conclusion The combined detections of NOB- 1 and CyclinD1 may be of clinical value in evaluating biological behavior and prognosis of thyroid papil ary carcinoma.%目的:探讨NOB-1和CyclinD1在甲状腺乳头状癌中联合表达的意义。方法采用免疫组化Envision二步法染色检测68例甲状腺乳头状癌,30例结节性甲状腺肿和30例同期癌旁3~5cm以外的正常甲状腺组织中NOB-1和CyclinD1的表达。结果在正常甲状腺组织、结节性甲状腺肿、甲状腺乳头状癌组织中:NOB-1阳性率分别为6.67%、13.33%、77.94%;CyclinD1阳性率分别为0.00%、20.00%、83.82%,两种蛋白阳性率在不同组织中差异均有统计学意义(均P<0.01)。在甲状腺乳头状癌组织中NOB-1阳性率Ⅲ、Ⅳ期(91.43%)高于Ⅰ、Ⅱ期(63.64%);有淋巴结转移(96.00%)高于无淋巴结转移(67.44%);原发灶肿瘤有侵犯包膜的(95.65%)高于无侵犯包膜的(40.91%),差异均有统计学意义(P<0.05或0.01);CyclinD1的阳性率在Ⅲ、Ⅳ期(94.29%)高于Ⅰ、Ⅱ期(72.73%);有淋巴结转移(100.00%)高于无淋巴结转移(58.88%);原发灶肿瘤有侵犯包膜的(97.83%)高于无侵犯包膜的(54.55%),差异均有统计学意义(P<0.05或0.01)。并且随着肿瘤的进展,NOB-1和CyclinD1阳性率均增高,两者出现高度的一致性,呈正相关(r=0.344,P<0.01)。结论 NOB-1和CyclinD1在甲状腺乳头状癌中呈过表达,两者联合检测对于评估甲状腺乳头状癌生物学行为及预后具有重要的临床意义。
