芪苈强心胶囊改善糖尿病心力衰竭大鼠心功能及血管内皮功能实验研究

         

摘要

Objective To evaluate the effects of Chinese traditional medicine Qili Qiangxin Jiaonang on cardiac function and vascular endothelial function in diabetic rats. Methods Forty-one SD rats were randomly divided into control group(n=7),diabetic group(n=18),and Qili Qiangxin group(n=16). The diabetic mellitus model was induced by injection of STZ in diabetic group and Qili Qiangxin group. Rats in Qili Qiangxin groups were gavaged with Qiliqiangxin on day 1 after successfully modeling(RBG>16.7mmol/L) for 8 weeks,and rats in other two groups were given saline. After 8-week intervention,the left ventricular diastolic diameter(LVIDD),left ventricular systolic diam-eter(LVIDS),left ventricular eject fraction(EF),and fraction shortening(FS) were evaluated by echocardiography and left ventricular end systolic pressure (LVSP),left ventricular end diastolic pressure (LVEDP),+dp/dt max,-dp/dt max,and cardiac index (heart weight/body weight) were assessed by catheter. Vascular endothelial function was measured by the tension of isolated thoracic aorta with endothelium dependant acetylcholine response test. Results Compared with control group,rats' body weightwas decreased,cardiac index was increased,and heart rate reduced in diabetic group (Ps<0.01). No significant difference in body weight,cardiac index,and heart rate was seen between diabetic group and Qili Qiangxin group. Ultrasonograph showed that LVIDD,EF and FS was significantly reduced in diabetic group compared with those in control group(Ps<0.01) and that LVIDS was decreased(P<0.05) and EF and FS were increased (P<0.05,P<0.01) in Qili Qiangxin group compared with diabetic group,but no change in LVIDD was seen between two groups. Hemodynamic analysis revealed that +dp/dtmax was down-regulated in diabetic group compared with that in control group(P<0.01),but no significant difference was found in LVSP,LVEDP,and -dp/dt-max. The down-regulation of +dp/dtmax was reversed by Qili Qiangxin(P<0.01),but no effect on LVSP,LVEDP and-DP/dtmax. Compared with control group,acetylcholine-mediated vasodilation rate was decreaed in diabetic group (P<0.05);compared with diabetic group,the decrease in vasodilation rate was less in Qili Qiangxin group (P<0.05). Conclusion Qiliqiangxin Jiaonang can improve vascular endothelial relaxation function in diabetic rats. This might be one of the mechanisms that causes the restoration of left ventricular systolic and diastolic function in rats with heart failure.%目的:观察芪苈强心胶囊改善糖尿病大鼠心功能与血管内皮功能的疗效。方法雄性SD大鼠随机分为对照组7只,糖尿病组18只,芪苈强心组16只。糖尿病组和芪苈强心组大鼠腹腔注射链脲佐菌素(STZ)诱导糖尿病模型,造模成功后(随机血糖>16.7mmol/L)1天即开始分别给予生理盐水和芪苈强心灌胃,对照组给予生理盐水灌胃。灌胃8周后测定各组大鼠体质量、左室舒张末期内径(LVIDD)、左室收缩末期内径(LVIDS)、射血分数(EF)、左室短轴缩短率(FS),血流动力学检查测左室收缩峰压(LVESP)、左室舒张末压(LVEDP)、左室内压最大上升(+dp/dtmax)与下降速率(-dp/dtmax),及心脏质量指数(心脏质量/体质量)。观察离体胸主动脉环对内皮依赖性扩血管物质乙酰胆碱的舒张反应。结果①一般情况:与对照组比较,糖尿病组大鼠体质量明显减轻(P<0.01),心脏质量指数明显增高(P<0.01),心率变慢(P<0.01)。与糖尿病组比较,芪苈强心组大鼠体质量、心脏质量指数、心率无改变。②心脏超声检测:与对照组比较,糖尿病组LVIDD明显减小(P<0.05), EF、FS降低(P均<0.01);与糖尿病组比较,芪苈强心组LVIDD无明显变化,但LVIDS明显减小(P<0.05),EF、FS增高(P<0.05,P<0.01)。③血流动力学检测:与对照组比较,糖尿病组+dp/dtmax明显下降(P<0.01),LVSP、LVEDP、-dp/dtmax差异无统计学意义(P>0.05);与糖尿病组比较,芪苈强心组+dp/dtmax明显增高(P<0.01),LVSP、LVEDP、-dp/dtmax差异无统计学意义(P>0.05)。④胸主动脉环对乙酰胆碱反应性测定:与对照组比较,糖尿病组胸主动脉环对乙酰胆碱介导的舒张百分比降低(P<0.05);芪苈强心组乙酰胆碱介导的舒张反应降低少于糖尿病组(P<0.05)。结论芪苈强心胶囊能改善糖尿病大鼠血管内皮舒张功能,这可能是其改善心力衰竭大鼠左室收缩功能的作用机制之一。
