调肺运脾化痰祛湿法治疗咳嗽变异性哮喘临床疗效

         

摘要

目的 观察调肺运脾化痰祛湿法治疗咳嗽变异性哮喘(肺脾亏虚型)的效果,探讨其对细胞间粘附分子(ICAM-1)和T淋巴细胞亚群的影响.方法 选择2016年9月至2018年4月咳嗽变异性哮喘(肺脾亏虚型)患者90例,依据随机数字表法分组.对照组45例采用西医常规治疗,观察组45例在对照组的基础上联合中药调肺运脾化痰祛湿法治疗,12周为1个疗程,治疗后比较两组炎症指标、免疫功能及肺功能改善情况,比较两组临床疗效.结果 治疗后观察组用力肺活量(FVC)、第一秒用力呼出量/用力肺活量(FEV1/FVC)大于对照组,差异有统计学意义(P<0.05);治疗后观察组白介素-6(IL-6)及 ICAM-1低于对照组(P<0.05);治疗后观察组CD4+/CD8+T、CD4+T、CD8+T等免疫功能指标高于对照组(P<0.05);观察组总有效率95.56%,高于对照组77.78%,差异有统计学意义(P<0.05).结论 联合调肺运脾化痰祛湿法治疗咳嗽变异性哮喘患者效果显著,可改善肺功能,减轻炎症反应,调节免疫功能,值得推广.%Objective To observe the effect of regulating lung and spleen,resolving phlegm and removing dampness on cough variant asthma (lung-spleen deficiency type),and to explore its effect on intercellular adhesion molecule(ICAM-1)and T lymphocyte subsets. Methods 90 patients with cough variant asthma(lung and spleen deficiency type)from September 2016 to April 2018 were selected and divided into groups according to random number table. 45 cases in the control group were treated with routine western medicine,45 cases in the observation group were treated with the method of regulating lung,promoting spleen,resolving phlegm and removing dampness,12 weeks as a course of treatment. After treatment, the inflammatory indexes,immune function and pulmonary function of the two groups were compared,and the clinical efficacy of the two groups was compared. Results After treatment,FVC,FEV1/FVC in the observation group were higher than those in the control group(P<0.05),IL-6 and ICAM-1 in the observation group were lower than those in the control group(P<0.05),and CD4+/CD8+T,CD4+T and CD8+T in the observation group were higher than those in the control group(P<0.05). The effective rate was 95.56%,higher than 77.78% in the control group(P<0.05). Conclusion The combined therapy of regulating lung,promoting spleen,resolving phlegm and removing dampness is effective in treating cough variant asthma. It can improve lung function,alleviate inflammation and regulate immune function. It is worth popularizing.
