胆囊神经内分泌癌的CT表现

摘要

目的 探讨胆囊神经内分泌癌的CT表现.方法 回顾性分析5例经手术病理证实的胆囊神经内分泌癌的增强CT表现.结果 5例患者中,表现为肿块型2例,胆囊壁增厚型3例;病灶平扫呈等、低密度,增强扫描见持续性均匀/不均匀中度强化;3例病灶内可见坏死.其中4例痛变边界清晰,所有5例病变黏膜面均存在部分连续细线样强化.2例患者存在局部肝脏侵犯;4例患者存在腹腔、腹膜后多发淋巴结转移,其中3例淋巴结最大短径＞2cm.结论 胆囊神经内分泌癌的增强CT表现(病灶边界、黏膜线、淋巴结转移等)存在一定特征性,对其诊断与鉴别有一定价值.%Objective To investigate the CT findings of gallbladder neuroendocrine carcinomas.Methods In 5 patients with gallbladder neuroendocrine carcinomas confirmed by pathology, the features on contrast-enhanced CT were retrospectively analyzed.Results Two cases were space replacing masses, and 3 cases presented as focal/diffuse wall thickening.Three cases showed heterogeneous enhancement with necrosis in the lesion, while the other 2 cases were moderate homogeneous enhancement.Lesions in all 5 patients showed entirely or partially continuous innermost mucosal layer on enhanced CT, and 4 cases had well-defined lesion margin.There were liver invasions in 2 cases and abdominal lymph node metastases in 4 patients.In 3 cases, the diameters of lymphadenopathy were more than 2 cm.Conclusion CT findings of gallbladder neuroendocrine carcinomas are relatively specific.Lesion margin, the presence of continuous innermost mucosal layer and predominately enlarged lymph nodes may help in the diagnosis of this disease.
