体位管理对妇科腹腔镜手术患者机体应激及心理状态的影响

摘要

目的 研究及分析体位管理对妇科腹腔镜手术患者机体应激及心理状态的影响情况.方法 选取2013年10月至2015年12月66例妇科腹腔镜手术患者为观察对象,将其随机分为A组(40°倾斜组)33例和B组(20°倾斜组)33例,然后将两组患者的术前即刻、术中15min、30min及术后即刻的血清应激激素、循环及呼吸指标、术前即刻及术后即刻、术后12h的HAD量表评分进行比较.结果 术前即刻两组患者的血清应激激素、循环及呼吸指标、HAD量表评分均无明显差异(P均>0.05),而术中15min、30min及术后即刻B组的血清应激激素、循环及呼吸指标均低于A组,术后即刻、术后12h的HAD量表评分均优于A组,两组间的差异均有统计学意义(P均<0.05).结论 体位管理对妇科腹腔镜手术患者机体应激及心理状态的影响较大,而20°倾斜体位对此类患者的影响更为积极,因此其应用价值相对更高.%Objective To study and analyze the influence situation of position management for the body stress and psychology state of patients with laparoscopic operation in Department of Gynaecology. Methods 66 patients with laparoscopic operation in Department of Gynaecology from October 2013 to December 2015 were selected as the study object,and they were randomly divided into group A(40 degrees of tilt group)of 33 cases and observation group(20 degrees of tilt group) of 33 cases,then the serum stress hormones,circulatory and respiratory parameters at once before the operation and at 15min,30min during the operation,at once after the operation,HAD scale at once before the operation,at once after the operation,at 12h after the operation of two groups were compared. Results The serum stress hormones,circulatory and respiratory parameters, HAD scale of two groups at once before the operation all had no obvious differences,all P>0.05,while the serum hormones,circulatory and respiratory parameters of group B at 15min,30min during the operation,at once after the operation were all lower than those of group A,the HAD scale at once after the operation and at 12h after the operation were all better than those of group A,all P<0.05,the differences of two groups were all significant. Conclusion The influence of position management for the body stress and psychology state of patients with laparoscopic operation in Department of Gynaecology are great,while the influence of 20 degrees of tilt for the patients are more active,so its application value is higher.
