目的：对比研究一期切除吻合术和传统二期吻合术治疗左半结肠癌肠梗阻的临床疗效优劣差异。方法选取2012年10月—2016年月2月期间收治入该院的左半结肠癌肠梗阻患者40例，按照就诊ID号随机分为观察组与对照组两组各20例，对照组实行传统二期吻合术，观察组实行一期切除吻合术，比较两组患者手术相关指标以及术后1年生存率。结果两组患者手术相关指标比较，手术时长﹑术中出血量以及术后肛门排气时间比较差异无统计学意义(P>0.05)，观察组患者住院时间(10.6±2.5)d和并发症发生率(5.0%)显著低于对照组患者(13.2±1.1)d和并发症发生率(30.0%)，差异有统计学意义(P<0.05)；术后1年生存率比较，观察组生存率(95.0%)显著高于对照组(65.0%)，差异有统计学意义(P<0.05)。结论左半结肠癌肠梗阻应用一期切除吻合术治疗安全有效，可降低患者住院时间和并发症发生情况，提高患者术后生存率，在临床治疗中有较高推广应用价值。%Objective To investigate the clinical difference between one-stage resection anastomosis and two-stage re-section anastomosis in the patients with left colon cancer complicated with intestinal obstruction. Methods 40 parents with left colon cancer complicated with intestinal obstruction between October 2012 to February 2016 were chose and randomly divided into control group and observation group according to the ID number.The control group were treated with two-stage resection anastomosis,the observation group were treated with one-stage resection anastomosis.The oper-ation indicators and 1 year survival rate were selected to compare. Results The bleeding volume,surgery time and post-operative exhaust time of two groups had no significant differences (P> 0.05),The hospitalization time (10.6±2.5)d and incidence of complications 5.0% in the observation group were significantly lower than the hospitalization time(13.2± 1.1)d and incidence of complications 30.0% in the control group,and 1 year survival rate in observation group(95.0%) were significantly better than that of control group (65.0%)(P<0.05). Conclusion The application of one-stage resec-tion anastomosis is an effective and safe treatment for left colon cancer complicated with intestinal obstruction,which can improve the incidence of complications, increase survival rate.
