闭合复位经皮空心螺钉内固定与切开复位内固定治疗踝关节骨折疗效对比研究

摘要

Objective:To compare and analyze the effect of closed reduction and percutaneous cannulated screw fixation and open reduction and internal fixation in the treatment of ankle fractures.Methods:129 cases of ankle fractures were selected.They were divided into closed reduction group (68 cases) and open reduction group (61 cases) according to different surgical methods.The closed reduction group was treated with closed reduction and percutaneous cannulated screw fixation, and the open reduction group was treated with open reduction and internal fixation.The improvement of ankle function, intraoperative and postoperative conditions were compared between the two groups before and after 1 months, 3 months and 6 months.Results:The total effective rate of closed reduction group (95.59％) was higher than that of open reduction group (80.33％) (P<0.05).A score of 1 months, 3 months and 6 months of ankle joint function after operation in two groups were higher than those before (P<0.05) ;closed reduction group after 1 months, 3 months and 6 months, the ankle score higher than the same period of open reduction group (P<0.05).The amount of bleeding in the closed reduction group was less than that in the open reduction group, and the operation time was shorter than that of the incision reduction group, and the healing time of the fracture was faster than that in the incision reduction group (P<0.05).The incidence of postoperative complications in the closed reduction group (1.47％) was lower than that in the incision reduction group (19.67％) (P<0.05).Conclusion:Closed reduction and percutaneous cannulated screw fixation is better than open reduction and internal fixation in the treatment of ankle fracture.%目的:对比分析闭合复位经皮空心螺钉内固定与切开复位内固定治疗踝关节骨折的效果.方法:踝关节骨折患者129例, 按照手术方法不同分为闭合复位组68例与切开复位组61例.闭合复位组采用闭合复位经皮空心螺钉内固定治疗, 切开复位组采用切开复位内固定治疗.比较两组疗效, 术前和术后1个月、3个月、6个月踝关节功能改善情况, 及术中和术后情况.结果:闭合复位组总有效率 (95.59％) 高于切开复位组 (80.33％), 且有统计学差异 (P<0.05) .两组术后1个月、3个月和6个月踝关节功能评分高于术前, 且有统计学差异 (P<0.05) ;闭合复位组术后1个月、3个月和6个月踝关节功能评分高于同期切开复位组, 且有统计学差异 (P<0.05) .闭合复位组术中出血量少于切开复位组, 手术时间短于切开复位组, 骨折愈合时间快于切开复位组, 均有统计学差异 (P<0.05) .闭合复位组术后并发症发生率 (1.47％) 低于切开复位组 (19.67％), 且有统计学差异 (P<0.05) .结论:闭合复位经皮空心螺钉内固定术治疗踝关节骨折患者效果优于切开复位内固定术.
