ABO血型正反定型不符的原因调查与分析

摘要

Objective:To review and analyze the test results of ABO blood group in hospital,summarize the reasons for the discrepancy of the forward and reverse ABO typing,and put forward to the solution. Methods:Microtubes gel method will be ap-plied to test the forward and reverse type of the ABO blood group for patients. Results:All 7 850 patients were examined for the forward and reverse typing of ABO blood group. The discrepancies happened in 20 cases of the forward and reverse ABO group type,which takes 0. 25% of the total number of detection. There were 9 cases of cold agglutination,2 cases of subtype,1 case of self - agglutination,4 cases of antibody weakening,1 case of congenitally non ABO antibody,and 2 cases of irregular anti-body due to blood transfusion or pregnancy,1 case of transfused with different blood type recently. Conclusion:For those ABO blood group cases with forward and reverse discrepancies,which couldn't be stereotyped by the existing technology in our hos-pital,we can adopt the gene detection method and other testing means to identify the ABO blood group correctly,so as to ensure the safety of the clinical blood transfusion.%目的：回顾分析山西省中医院 ABO 血型测定结果，总结造成 ABO 血型正反定型不符的原因，并提出解决方法。方法：采用微柱凝胶法和试管法对住院及门诊患者进行 ABO 血型正反定型测定。结果：2014年1月-2016年1月住院及门诊患者7850例，其中 ABO 血型正反定型不符20例（0.25％）。其中冷凝集9例，亚型2例，自身凝集1例，抗体减弱4例，先天性无 ABO 抗体1例，因输血或妊娠发生不规则抗体2例，近期输过异型红细胞1例。结论：对ABO 血型正反定型不符，山西省中医院现有技术手段无法定型的 ABO 血型样本，可以采用基因检测法等其他检测手段正确鉴定 ABO 血型，从而保证临床输血的安全性。
