血清同型半胱氨酸水平异常与脑小血管病的相关性研究

         

摘要

目的探讨血清同型半胱氨酸(Hcy )水平异常与脑小血管病(CSVD )的相关性。方法连续收集204例脑小血管病患者血清同型半胱氨酸及临床资料,其中腔隙性梗死(LI)103例,脑白质病变(WML )101例,以同期健康体检者56例为对照,研究血清同型半胱氨酸水平异常与脑小血管病的相关性。结果 CSVD患者血清Hcy水平[(17.24±7.37)μmol/L]显著高于对照组[(13.70±7.08)μmol/L],差异有统计学意义( P<0.01)。WML组Hcy水平[(18.66±7.06)μmol/L]显著高于LI组[(16.14±7.24)μmol/L],差异有统计学意义( P<0.01)。Logistic回归分析显示Hcy水平与CSVD发病密切相关(OR=1.11,P<0.01),Hcy水平与WML及LI发病密切相关(OR=1.10、1.07,P<0.05)。结论血清同型半胱氨酸水平异常与脑小血管病具有显著相关性,且在脑白质病变组更显著。%Objective To investigate the relationship between serum homocysteine level and cerebral small vessel disease (SVD ) . Methods 204 cases who diagnosed as CSVD were consecutively enrolled into this study .The patients with CSVD were further divided in-to lacunar infarction (LI) group with 103 cases and white matter lesion (WML) group with 101 cases .56 healthy cases were enrolled as the normal control group .Then the relationship between serum homocysteine level and cerebral small vessel disease was investigated . Results Hcy level in CSVD was [(17 .24 ± 7 .37)μmol/L] ,which was significantly higher than the control group of [(13 .70 ± 7 .08)μmol/L] (P<0 .01) .Hcy level in WML was [(18 .66 ± 7 .06)μmol/L] and was significantly higher than in LI [(16 .14 ± 7 .24)μmol/L] (P<0 .01) .Logistic regression analysis showed that Hcy levels was closely related with CSVD ,(OR=1 .11 , P<0 .01) . Hcy levels was closely related with WML and LI ,(OR=1 .10、1 .07 , P<0 .05) .Conclusions There was a significant correlation be-tween homocysteine and cerebral small vessel disease ,it is more significant in the cerebral white matter lesion group .
