目的：探讨生物型人工股骨头置换治疗高龄股骨颈骨折的临床疗效。方法我院骨科采用生物型人工股骨头置换治疗的高龄股骨颈骨折患者102例，观察患者住院时间、手术时间、出血量、术后并发症、开始下床活动时间、髋关节功能等指标。结果本组手术时间45～70分钟，术中出血150～350 ml，所有患者均安全度过围手术期，无死亡病例，无切口感染及神经血管损伤病例。术后4例发生下肢深静脉栓塞，3例发生尿路感染，8例发生肺部感染，均经积极治疗后痊愈。住院时间15～28天，开始下床活动时间为术后10～18天。89例患者获得1～5年的随访，优56例，良31例，中2例，优良率97．75％，Harris髋关节功能评分与治疗前比较，差异有统计学意义（ P＜0．01）。结论生物型人工股骨头置换术操作简便，患者易耐受，临床效果好，是治疗高龄股骨颈骨折的理想方法之一。%Objective To investigate the clinical effect of artificial femoral head replacement in the treatment of femoral neck fracture in the elderly.Methods One hundred and two elderly patients with femoral neck fracture treated with artificial femoral head replacement in our department were involved in the study.The length of hospital stay, operation time, blood loss, postoperative com-plications, time of starting out of bed, hip function and other indicators were observed.Results The operation time was 45~70 mi-nutes, and the bleeding during operation was 150~350 ml.All patients were safe through the perioperative period, and there was no death, incision infection and nerve vascular injury.There were 4 patients with postoperative deep vein thrombosis, 3 patients with uri-nary tract infection, and 8 cases with pulmonary infection.These postoperative complications were cured by active treatment.Hospital stay was 15~28 days.The postoperative activity time of began to get out of bed was 10~18 days.Of these patients, 89 patients were followed up for 1~5 years.According to the Harris hip function score, 56 cases were excellent, 31 were good and 2 cases were medi-um.The excellent and good rate was 97.75%.The difference in Harris hip function score before and after the treatment was statistical-ly significant (P<0.01).Conclusion Artificial femoral head replacement is a simple and effective method for the treatment of femo-ral neck fracture in the elderly.
