18F-FDGPET/CT对中枢神经系统原发性肿瘤复发的诊断价值

         

摘要

目的 探讨18F-脱氧葡萄糖-正电子发射断层扫描 (18F-FDGPET/CT) 对中枢神经系统原发性肿瘤复发的诊断价值.方法 对80例已获病理学证实但术后疑为复发的中枢神经系统原发性肿瘤患者行18F-FDGPET/CT检查, 包括CT平扫及PET发射扫描, 扫描范围自颅顶至腹股沟下, 获得横断、矢状、冠状面CT、PET及PET/CT融合图像.结果 80例患者证实为肿瘤复发47例, 放疗后组织纤维化或坏死33例, 复发肿瘤Ⅰ级12例, Ⅱ级21例, Ⅲ级7例.18F-FDG-PET/CT诊断为真阳性38例, 真阴性27例, 假阳性6例, 假阴性9例, 敏感度为80.9%, 特异度为81.8%, 阳性预测值为86.4%, 阴性预测值为75.0%.结论 18F-FDGPET/CT检查中枢神经系统原发性肿瘤复发比较准确可靠.%ObjectiveTo evaluate the value of18F-fluorodeoxyglucose positron emission tomography (18F-FDGPET/CT) in the diagnosis of recurrent primary tumor of the central nervous system.Methods 80 patients with primary tumors of the central nervous system who had been confirmed by pathology but suspected of recurrence were conducted18F-FDGPET/CT examination, including CT scan and PET scan., scan range from skull top to groin, transverse, sagittal, coronal plane, CT, PET and PET/CT fusion images were obtained.Results Among80 patients, 47 cases were proved to tumor recurrence, 33 cases were proved to fibrosis or necrosis after radiotherapy, 12 cases were grade I, 22 cases were gradeⅡ, 7 cases were gradeⅢin the patients with tumor recurrence.38 cases were diagnosised true positive by18F-FDGPET/CT, 27 cases were diagnosised true negative, 6 cases were diagnosised false positive, 9 cases were diagnosised false negative.The sensitivity of18F-FDGPET/CT was 80.9%, and the specificity was 81.8%, and the positive predictive value was 86.4%, and the negative predictive value was 75.0%.Conclusion18F-FDGPET/CT examination of central nervous system primary tumor recurrence is relatively accurate and reliable.
