目的：探讨超声对绝经后妇女卵巢畸胎瘤的诊断价值。方法选择进行诊治的绝经后妇女卵巢畸胎瘤60例，手术病理确诊为成熟畸胎瘤45例，未成熟畸胎瘤15例，对常规超声与彩色多普勒超声指标进行分析。结果与成熟畸胎瘤对比，未成熟畸胎瘤多表现为多发实性肿物，肿瘤内部有明显血流信号，瘤体直径比较大。未成熟畸胎瘤的卵巢动脉血流峰值速度与血流量明显低于成熟畸胎瘤，而阻力指数明显高于成熟畸胎瘤，对比差异都有统计学意义（P＜0．05）。经过超声判断为未成熟畸胎瘤16例，成熟畸胎瘤44例。超声对绝经后妇女卵巢畸胎瘤的诊断敏感性与特异性为97．8％和100．0％。结论超声诊断绝经后妇女卵巢畸胎瘤能通过特征形态与卵巢动脉血流的观察提高诊断敏感性与特异性，有很好的诊断价值。%Objective To evaluate the diagnostic values of ultrasonography in postmenopausal women with ovarian tera -toma.Methods 60 postmenopausal women with pathologically confirmed ovarian teratoma were selected , and 45 patients were confirmed to be mature teratoma , 15 patients were confirmed to be immature teratoma , the conventional ultrasound and color doppler echocardiography were analyzed .Results Compared with mature teratoma ,immature teratoma were mainly multiple solid tumor,there were significant blood flow signals within the tumor ,tumor diameter were relatively large .The ovarian artery peak ve-locity and blood flow in the immature teratoma were significantly lower than that of the mature teratoma ,and the resistance index were significantly higher than that of mature teratoma ,there had statistically significant differences (P<0.05).After ultrasound judged as immature teratoma were 16 cases,44 cases were mature teratoma ,diagnostic sensitivity and specificity of ultrasound for postmenopausal women with ovarian teratoma were 97.8%and 100.0%.Conclusion Ultrasound in the diagnosis of postmeno-pausal women with ovarian teratoma can improve diagnostic sensitivity and specificity by ovarian morphology and arterial blood flow,it has good diagnostic value .
