目的 探讨回肠袢式造口术和结肠造口术治疗老年梗阻性乙状结肠癌的效果.方法 将70例老年梗阻性乙状结肠癌患者随机分为研究组和对照组,各35例.研究组患者给予回肠袢式造口术,对照组患者给予结肠造口术,观察2组患者的临床疗效及安全性.结果 研究组的治疗有效率为77.14%,对照组为65.71%,差异无统计学意义(P>0.05);研究组的控制率为94.29%,高于对照组的85.71%,差异具有统计学意义(P<0.05).Ⅰ期手术时2组患者手术时间、住院时间差异不大,但研究组患者禁食时间显著少于对照组(P<0.05);Ⅱ期手术研究组患者手术时间、禁食时间、住院时间均显著少于对照组,组间比较差异具有统计学意义(P<0.05).2组患者分期手术过程中并发症的发生率差异不大(P>0.05).结论 对老年梗阻性乙状结肠癌患者使用回肠袢式造口术治疗相对于结肠造口术来说有较大的优势,利于老年患者康复,可推广使用.%Objective To investigate the therapeutic effect of ileal loop ostomy and colostomy in elderly patients with ob-structive sigmoid colon cancer. Methods 70 cases of elderly patients treated with obstructive sigmoid colon cancer were random-ly divided into the study group and the control group (n=35). The patients in the study group were given ileal loop oral opera-tion,and the control group of patients given colostomy, while the clinical efficacy and safety of the 2 groups patients were ob-served. Results The effective rate of the study group was 77. 14% and the control group was 65. 71%,the difference between the 2 groups was not statistically significant (P>0. 05). The control rate of the study group was 94. 29%,which was higher than the control group(85. 71%),there was statistically significant difference between the 2 groups(P<0. 05). Operation and hospi-talization time of the 2 groups in the stageⅠhad no significant difference(P>0. 05). But the patients' fasting time of the study group was significantly lower than the control group(P<0. 05). In the stageⅡ,operation time,hospitalization time and fasting time of the study group were significantly lower than the control group(P<0. 05). There was no significant difference in the inci-dence of complication between the 2 groups(P>0. 05). Conclusion Ileal loop ostomy has greater advantage than colostomy for elderly patients with obstructive sigmoid colon cancer,and it is beneficial to the rehabilitation of elderly patients.
