Bcl -2和PD -L1在大肠癌组织中的表达及两者与大肠癌侵袭转移的相关性

摘要

Objective To study the expression of Bcl - 2 and PD - L1 in colorectal cancer tissue, and to investigate their relationship with clinical pathological features and the correlation with invasion and metastasis of colorectal cancer. Methods Bcl - 2 and PD - L1 were detected immunohistochemically in 57 cases of colorectal cancer tissue specimens and normal tissue samples, then correlation analysis were taken combined with clinical and pathological features. Results (Dthe positive expression rates of Bcl - 2 and PD - L1 in colorectal cancer tissues were higher than their expression in normal mucosa tissues, and there were significant differences - ( P ＜ 0. 05 ). ②The expression of BCL - 2 and PD - L1 in colorectal cancer with the high and the moderately - differentiated group were significantly increased compared with the poorly - differentiated group, there was significant difference ( P ＜ 0. 01). The positive expression rate of PD - L1 in colorectal cancer patients with lymph node metastasis ( Dukes stage C + D) was elevated significantly compared to the group without lymph node metastasis (Dukes stage A + B) (P ＜0. 05).③In the high - and the moderately - differentiated colorectal cancer, Bcl -2 expression rate in PD - L1 negative group was higher than that in PD - L1 positive group, there was significant difference ( P ＜0. 01). Conclusions The detection of Bcl - 2 and PD - L1 expression in cobrectal cancer can help determine the ability of invasion and metastasis and may have important clinical significance fur the diagnosis, treatment and prognosis judgment of colorectal cancer.%目的 分析Bcl -2及PD - L1在大肠癌中的表达并探讨其与大肠癌临床病理特征之间的关系,及与大肠癌侵袭转移的相关性.方法 用免疫组化法检测57例手术切除大肠癌组织新鲜标本及癌旁正常黏膜组织中Bcl -2及PD - L1的表达,并结合临床病理特征进行相关性分析.结果 ①与癌旁正常组织相比,Bcl -2及PD - L1在大肠癌组织中的表达明显升高,有统计学差异(P＜0.05).②Bcl -2及PD - L1在大肠癌患者中高-中分化组与低分化组相比,表达明显升高,有显著性差异(P＜0.01)；PD - L1在大肠癌患者中有淋巴结转移组(Dukes分期C+D)与无淋巴结转移组(Dukes分期A+B)相比,表达升高,有统计学差异(P＜0.05).③在高-中分化的大肠癌组织中,PD - L1阴性组中Bcl -2阳性表达明显高于PD - L1阳性组,有显著统计学差异(P＜0.01).结论 联合检测大肠癌组织中Bcl -2与PD - L1的表达,能协助判断大肠癌侵袭转移的能力,对大肠癌的诊断、指导治疗及评价预后有重要的临床意义.
