GrB+淋巴细胞、T-bet+淋巴细胞和CD68+巨噬细胞的表达与乳腺癌侵袭能力和预后的关系

摘要

目的 探讨GrB+淋巴细胞、T-bet+淋巴细胞和CD68+巨噬细胞的表达与乳腺癌侵袭能力和预后的关系.方法 选择125例乳腺癌患者的乳腺癌组织及其对应的癌旁组织,采用RT-PCR法检测乳腺癌组织及癌旁组织中GrB mRNA、T-bet mRNA和CD68 mRNA的表达情况,分析其与乳腺癌侵袭能力和预后的关系.结果GrB mRNA在乳腺癌上皮组织和乳腺癌间质组织中的表达水平高于乳腺癌癌旁组织(P﹤0.05);T-bet mRNA和CD68 mRNA在乳腺癌癌旁组织中的表达水平高于乳腺癌上皮组织和乳腺癌间质组织(P﹤0.05).GrB+淋巴细胞高表达组患者的1年、3年和5年累积无复发生存率分别为66.67%、55.56%和42.86%,GrB+淋巴细胞低表达组患者的1年、3年和5年累积无复发生存率分别为45.16%、33.87%和22.58%,差异均有统计学意义(P﹤0.05).多因素分析结果显示,肿瘤最大径﹥5 cm、淋巴结转移、孕激素受体阳性以及GrB mRNA低表达是影响乳腺癌侵袭能力和患者无复发生存率的危险因素(P﹤0.05);T-bet mRNA和CD68 mRNA低表达是影响乳腺癌患者生存预后的独立危险因素(P﹤0.05).结论 GrB mRNA、T-bet mRNA和CD68 mRNA低表达是影响乳腺癌患者生存预后的独立危险因素.%Objective To evaluate the relationship between the expression of GrB+,T-bet+and CD68+macrophage and the invasion and prognosis of breast cancer.Method The expression of GrB mRNA,T-bet mRNA and CD68 mRNA in the breast cancer tissues and adjacent tissues of 125 cases of breast cancer patients were measured by RT-PCR.The re-lationship between the expression of GrB,T-bet and CD68 mRNA and the invasion ability and prognosis was investigat-ed.Result The expression of GrB mRNA was significantly higher in epithelial and interstitial tissues than in adjacent tis-sues of breast cancer(P<0.05);while T-bet and CD68 mRNA were highly expressed in adjacent tissues than in epithelial and interstitial tissues in these patients(P<0.05).The 1-,3-,and 5-year cumulative recurrence free survival rate in pa-tients with high expression of GrB+lymphocytes were 66.67%,55.56%,and 42.86%,while that in patients with low ex-pression of GrB+were 45.16%,33.87%,and 22.58%,respectively,the differences were statistically significant(P<0.05). Multivariate analysis showed that maximum diameter of tumor>5 cm,lymph node metastasis,positive expression of pro-gesterone receptor and low expression of GrB mRNA were risk factors of the invasion ability and free survival rate for breast cancer patients(P<0.05);low expression of T-bet and CD68 mRNA was the risk factor for the survival and progno-sis in breast cancer patients(P<0.05).Conclusion Low expression of GrB,T-bet and CD68 mRNA are significantly cor-related with the prognosis of breast cancer patients.
