目的：探讨犀角地黄汤联合匹多莫德治疗儿童过敏性紫癜（ HSP）的临床效果及免疫功能的影响。方法将174例HSP患儿随机分为研究组和对照组各87例，对照组在常规治疗的基础上加用匹多莫德口服液治疗，研究组在对照组的基础上加用犀角地黄汤治疗。检测2组治疗前后T淋巴细胞亚群、免疫球蛋白、干扰素－γ（ INF－γ）、白细胞介素－10（ IL－10）水平，观察2组6个月、12个月复发率及治疗期间的不良反应。结果研究组12个月复发率低于对照组（ P＜0．05）。治疗后研究组CD4＋T细胞、CD4＋／CD8＋T细胞比值、IgG水平、IFN－γ水平均较治疗前显著升高（P均＜0．05），且研究组均较对照组显著升高（P均＜0.05）。治疗后研究组CD8＋T细胞、IgA和IgM水平、IL－10水平均较治疗前显著降低（ P均＜0．05），且研究组较对照组显著降低（P＜0．05）。2组不良反应比较差异无统计学意义（P＞0．05）。结论犀角地黄汤联合匹多莫德治疗HSP可改善患儿免疫平衡，降低复发率，安全性高，具有临床推广价值。%Objective It is to explore the clinical effect of Xijiao Dihuang decoction combined with Pidotimod on Henoch -schonlein purpura ( HSP) and immune function in children .Methods 174 cases of children with HSP were randomly divided into study group and control group , each group had 87 cases.The control group was given Pidotimod oral liquid on the basis of normal therapy , while the study group was added with Xijiao Dihuang decoction additionally on the basis of therapy in control group.The levels of subsets of T -lymphocytes, immunoglobulin, INF-y, IL-10 before and after treatment were detected , the reoccurrence rate in 6, 12 months and side effect occurred during treatment stage were observed in both groups .Results The reoccurrence rate in 12 months in study group was lower than that in control group (P<0.05).The levels of CD4 +T cell, CD4 +/CD4+and IgG, IFN-y were increased significantly after treatment compared with that before treatment in both groups(P<0.05), and the increase in study group was more significant (P<0.05).The levels of CD8 +T cell, IgA and IgM, IL-10 were increased significantly after treatment compared with that before treatment in both groups (P<0.05), and the decrease in study group was more significant (P<0.05).There was no significant difference in side effect between the two groups(P>0.05).Conclusion Xijiao Dihuang decoction combined with Pidotimod could improve immune function , decrease reoccurrence rate with high safety in the treatment of HSP , it is worthy of population and application in clinic .
