心肌肌钙蛋白I、肌钙蛋白T和NT-proBNP联合检测的诊断价值探讨

摘要

Objective To investigate the value combined detection of plasma cardiac troponin I,cardiac troponin T and NT-proBNP in acute myocardial infarction diagnose. Methods Collected 34 patients who have the with acute chest pain as the main symptom treatment from the department of emergency and cardiol-ogy clinic of Baoji Second People's Hospital since May 2012 to May 2013 for the study,the blood samples were drawn when the patients in the treatment,all the samples use electricity chemiluminescence method to detect the plasma cTnI, cTnT, NT-proBNP concentrations. Results Plasma cTnI, cTnT, AMI NT-proBNP levels were higher than those in non AMI group,the difference was statistically significant(P<0. 01). The sensitivity of combined detection of three higher than those of the NT-proBNP test alone(χ2 =6. 23,P<0. 05),the specificity was higher than that of the cTnT test alone(χ2 =9. 38,P<0. 05). Conclusion NT-proBNP,cTnI,cTnT combined detection of AMI detection sensitivity can be improved to help early diagnosis of AMI.%目的：探讨血浆中肌钙蛋白I( cTnI)、肌钙蛋白T( cTnT)、N端前B型脑钠肽( NT-proBNP)的联合检测在急性心肌梗死( AMI)诊断中的应用价值。方法收集2012年5月至2013年5月宝鸡市第二人民医院收治的以急性胸闷胸痛为主要症状就诊的34例患者为研究对象,以2010年AMI临床诊断与治疗指南( WHO)为金标准将这34例患者分为AMI组20例和非AMI组14例,所有患者在就诊时即抽取血样,用电化学发光法测定两组患者血浆中的cTnI、cTnT、NT-proBNP水平,并分析cT-nI、cTnT和NT-proBNP单独及联合检测诊断AMI的价值。结果 AMI组血浆cTnI、cTnT、NT-proBNP水平均高于非AMI组,差异有统计学意义(P<0.01)。三者联合检测的灵敏度高于NT-proBNP单独检测(χ2=6.23,P<0.05),特异度高于cTnT单独检测(χ2=9.38,P<0.05)。结论 NT-proBNP、cTnI、cTnT联合检测可以提高AMI检测的敏感性,有助于AMI的早期诊断。
