非感染患者术后低蛋白血症的发生机制及影响因素研究进展

         

摘要

Hypoproteinemia is a dose-dependent,repetitive and an independent risk factor for poor outcome in the critically ill patients.Postoperative plasma albumin is decreased in different levels,and the surgical ICU patients have a high incidence of hypoproteinemia.Hypoalbuminemia is closely associated with the illness severity and clinical outcomes,which can significantly increase the odds of mortality and morbidity,prolong ICU and hospital stay.Therefore,the clinical interventions aimed at relevant influencing factors of hypoalbuminemia in the critically ill should be emphasized,which plays a crucial role in reducing the mortality.At present,the pathogenesis of hypoalbuminemia has not yet been fully clarified,so the treatment of hypoproteinemia still remains controversial.%低蛋白血症是重症患者剂量依赖的、可重复的、独立的预后不良预测指标.手术后患者的血浆白蛋白水平均有不同程度的下降,术后入住外科ICU患者低白蛋白血症的发生率较高,低蛋白与病情严重程度及预后密切相关,其不仅能增加术后并发症的发生率及病死率,且能延长住ICU时间及总住院时间.及时发现并针对相关影响因素给予临床干预对降低病死率有至关重要的作用.然而目前导致术后低白蛋白血症的发生机制尚未完全阐明,是否需要白蛋白治疗仍存在争议.
