腰椎后路椎间植骨融合内固定术治疗复发性腰椎间盘突出症疗效分析

摘要

目的：分析腰椎间盘突出症术后同节段复发的原因，探讨腰椎后路椎间植骨融合内固定术治疗腰椎间盘突出症的适应证及临床疗效。方法：回顾性分析2008年1月-2011年9月使用腰椎后路椎间植骨融合内固定术治疗复发性腰椎间盘突出症患者的资料；采用JOA评分、VAS评分及影像学检查评价治疗效果。结果：23例患者均获得随访，时间6个月~3年，植骨块均融合，无机械不稳引起的并发症如椎体滑脱，无椎间融合器后退及下沉，无螺钉断裂及松动。结论：复发性腰椎间盘突出症原因尚难统一，腰椎的退变及术后患者的生活习惯是导致腰椎间盘突出症复发的重要原因，椎间融合器后路椎间融合内固定治疗符合生物力学要求，疗效确切，是提高患者生活质量的有效方法。%Objective:To study recurrent causes and treatment of postoperation on lumbar disc hernition. Method:23 recurrent cases underwent the pedicle screw fixation and lumbar interbody fusion. The postoperative data of all the cases were adopted for retrospective analysis. Result:All the patients were followed-up with a duration of 0.5-3 years postoperatively,no mechanical instability caused by complications such as vertebral olisthy,no interbody fusion cage back and sinking,no screw breakage and loosening. The patients obtained excellent or good result. Conclusion:The pedicle screw fixation and lumbar interbody fusion is effective in dealing with lumbar disc herniation recurrence after operation.
