炎症性肌病25-羟基维生素D水平及相关指标的临床意义

摘要

目的 探讨特发性炎症性肌病(IIM)血清25-羟基维生素D[25(OH)D]水平及其临床意义.方法 选取未经治疗的IIM患者35例,以44例其他自身免疫性疾病患者作为疾病对照组.检测所有患者的25(OH)D、白细胞(WBC)、中性粒细胞绝对数(NEUT#)、血红蛋白(Hb)、血小板(PLT)、肌酐(Cr)、尿酸(UA)、尿素氮(BUN)、丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)、肌酸激酶(CK)、肌酸激酶MB同工酶(CK-MB)、乳酸脱氢酶(LDH)、肌红蛋白(MYO)、心肌肌钙蛋白T(cTnT)、补体(C)3、C4、IgA、IgG、IgM、抗Jo-1抗体、抗核抗体,并对结果进行统计分析.结果 IIM组25(OH)D、Cr、UA、BUN水平均明显低于疾病对照组(P<0.05),ALT、AST、CK、CK-MB、LDH、MYO、C4水平及抗Jo-1抗体阳性率均明显高于疾病对照组(P<0.05),而性别、年龄、Hb、WBC、NEUT#、PLT、cTnT、IgA、IgG、IgM、C3及抗核抗体分型2个组之间差异均无统计学意义(P>0.05).结论 IIM患者血清25(OH)D水平明显低于其他自身免疫性疾病,可作为IIM与其他自身免疫性疾病鉴别诊断的辅助指标.%Objective To investigate the levels and roles of 25-hydroxyvitamin D [25(OH)D] levels and related indicators for idiopathic inflammatory myopathy(IIM).Methods A total of 35 untreated patients with IIM were enrolled,and 44 patients with other autoimmune diseases were enrolled as control group. The levels of 25(OH)D,white blood cell(WBC),neutrophil count(NEUT#),hemoglobin(Hb),platelet (PLT),creatinine(Cr),uric acid(UA),blood urea nitrogen(BUN),alanine aminotransferase (ALT),aspartate aminotransferase(AST),creatine kinase(CK),creatine kinase MB isoenzyme(CK-MB),lactate dehydrogenase(LDH),myoglobin(MYO),cardiac troponin T(cTnT),complement (C) 3,C4,IgA,IgG,IgM,anti-Jo-1 antibody and anti-nuclear antibody were determined. The results were analyzed statistically.Results The levels of 25(OH)D,Cr,UA and BUN were lower in IIM group than those in control group(P<0.05),and the levels of ALT,AST,CK,CK-MB,LDH,MYO and C4 and the positive rate of anti-Jo-1 antibody were higher in IIM group than those in control group(P<0.05). There was no statistical significance for sex,age,Hb,WBC,NEUT#,PLT,cTnT,IgA,IgG,IgM,C3 and anti-nuclear antibody between IIM and control groups(P>0.05).Conclusions The level of 25(OH)D in patients with IIM is lower than that in patients with other autoimmune diseases,which can be used as an auxiliary indicator for the differential diagnosis of IIM and other autoimmune diseases.
