速度向量成像技术评价新生儿窒息后左心室扭转功能

             

摘要

目的:应用速度向量成像技术(velocity vector imaging.VVI)评价新生儿窒息后左心室扭转功能,为临床提供可靠的诊疗指标.方法:应用VVI技术分别测量正常组和窒息组新生儿以及窒息组药物治疗后的左心室各扭转指标.规定从心尖向心底部观察逆时针旋转为正方向,反之为负;左室扭转等于心尖部与心底部旋转角度之差;左室扭力等于左室扭转角度除以舒张末期左室长度.结果:正常组及窒息组足月新生儿左心室心底与心尖的旋转方向均为正方向.正常组左心室心尖旋转角度3.193°±1.132°、心底旋转角度(2.410±1.530)°、左室扭转(1.698±1.010°)、左室扭力(0.061±0.036)°/min,窒息组新生儿左心室心尖旋转角度(2.001±1.069)°、左室扭转(1.226±0.717)°、左室扭力(0.044±0.269)°/min,明显低于正常组新生儿(P<0.05);但窒息组心底旋转角度(1.987±1.122)°,与正常组无明显差异(P>0.05).治疗后窒息组心功能基本恢复,但心尖旋转恢复稍差.结论:窒息后新生儿左心室扭转功能明显下降,VVI技术可以比较准确地对其进行评价,指导临床工作.%Objective To assess the value of echocardiography velocity vector imaging (VVI) on evaluating left ventricular twist function in post-asphyxia newborn infants. Methods The left ventricular twist function was detected by VVI in all infants in control group and infants in asphyxia group before and after treatment. The degree of left ventricular twist was equal to the difference between apical rotation and basal rotation. The strength of left ventricular twist was equal to the degree of left ventricular twist divided by the length of left ventricular in diastasis.Results The apical and hasal rotation was anticlockwise in the two groups. The apical rotation [ (3.193±1.132) °vs.( 2.001+1.069 ) ° ] , left ventricular twist [ ( 1.698±1.010) ° vs. ( 1.226±0.717 )° ] , left ventricular torsion [ ( 0.061±0.036)°/mm vs. 044±0.269) °/mm] of the post-asphyxia newborn infants were significantly lower than those of the normal group (P < 0.05). All parameters were back to normal after medical treatment. Conclusion The left ventricular twist function was injured , and VVI can ohjectively reflect left ventricular twist function , in the postasphyxia newborn infants.
