人工全髋关节置换术后假体松动的诊治及预防

摘要

Objective:To investigate the causes,treatment and prevention of prosthesis loosening after total hip arthroplasty(THA).Methods: 35 cases of prosthesis loosening after total hip arthroplasty in patients' clinical data were collected and analyzed,35 patients had pain side hip and groin area,some patients had anterior thigh and knee pain.X-ray showed continuous translucency around the prosthesis was greater than 3 cm,or prosthesis shift,surgery found false,acetabular prosthesis handle shift in the operation.4 cases with the fingers gently removed,soft tissue around the prosthesis showed light black.Cleared the light black tissue,removed the prosthesis,cleared bone cement,and reinstaledl the new artificial hip.Results: 35 cases of prosthesis loosening after total hip revision surgery,the pain disappeared,restore function,the result was good.Conclusion:The prosthesis loosening after THA is closely related to the patient's own cause,the choice of prosthesis and the operation of the operation,and early revision surgery should be targeted.%目的:探讨人工全髋关节置换术(total hip arthroplasty,THA)后假体松动的原因、诊治及预防.方法:收集分析35例全髋关节置换术后的假体松动患者的临床资料,35例患者均有患侧髋部及腹股沟区疼痛,部分病例出现大腿前方及膝部疼痛;X线表现为假体周围>3 cm的连续透光影,或假体发生移位,手术中发现假臼、假体柄移位.4例患者的假臼用手指轻轻取出,假体周围软组织呈现浅黑色.清除浅黑色组织,取出假体,清除骨水泥,重新安装新的人工全髋.结果:35例假体松动患者,经全髋翻修术后,疼痛消失,恢复功能,效果良好.结论:THA术后假体松动与患者自身原因、假体的选择和手术技术操作有密切关系,早期翻修手术要有针对性.
