E-cadherin和整合素α5β1的表达与结直肠癌生物学行为和预后的关系

摘要

目的:探讨E-cadherin、整合素α5β1的表达与结直肠癌生物学行为及预后的关系.方法:采用免疫组织化学染色法检测41例结直肠癌组织标本中E-cadherin和整合素α5β1的表达情况,另外取距癌灶大于10 cm的肠组织为正常对照.结果:E-cadherin在结直肠癌的正常表达率为51%,显著低于正常组织(100%,P<0.01).E-cadherin低表达组5年生存率为43%,正常表达组5年生存率为61%(P﹤0.05).在癌组织中整合素α5β1高表达的比例为61%,显著高于正常组织的20%(P<0.01);E-cadherin的表达与结直肠癌的Duke's分期和有无淋巴结转移有关,分期晚,E-cadherin低表达的比例高,且淋巴结有转移组低表达比例明显升高;整合素α5β1也与结直肠癌的Duke's分期和有无淋巴结转移有关,分期晚,整合素α5β1高表达的比例高(P<0.01),且淋巴结有转移组高表达比例明显升高(P<0.01);其高表达组5年生存率为41%,低表达组5年生存率为59%,比较差异有统计学意义(P<0.05).结论:E-cadherin及整合素α5β1对结直肠癌生物学行为均有明显影响,E-cadherin的正常表达将显著降低大肠癌的浸润和转移能力,患者预后较好;整合素α5β1高表达促进大肠癌浸润和转移,患者预后欠佳;E-cadherin和整合素α5β1在结直肠癌的发病机制中可能起着协同作用,两者的联合检测或许可以成为临床判断结直肠癌生物学行为及预后的重要参考指标.%Objective： To investigate the expression of E-cadherin and integrin α5β1 in colorectal cancer and their relationship with tumor biological behavior and patients' survival.Methods: Immunohistochemical staining of E-cadherin and integrin α5β1 were performed in 41 samples of colorectal carcinoma, and in normal colorectal mucosa samples obtained at 10 cm from the tumor edge as control.Results: The expression rate of E-cadherin in colorectal cancer was significantly lower than that in normal colon tissues (P ＜0.01).which was 49％ and 100％ , respectively.The five-year survial rate was significantly lower in patients with lower E-cadherin expression than in patients with normal E-cadherin expression (P ＜0.05) , which was 43％ , 61％ respectively.The expression of E-cadherin was related to the Dukes classification, and lymph node metastasis.The expression rate of integrin α5β1 was 61％ in colorectal cancer, which was higher than 20％ in normal colon tissues (P ＜0.01).The expression of integrin α5β1 also had closely relation to the Dukes classification and lymph node metastasis of colorectal cancer.The five-year survial rate was lower in patients with higher integrin α5β1 expression group than in patients with lower integrin α5β1 expression group ( P ＜0.05) , which was 41 ％ , 59％ respectively.The expression of integrin α5β1 had negative correlation with prognosis.Conclusion: E-cadherin and integrin α5β1 influence the colorectal cancer biology behavior obviously.The E-cadherin expression remarkably reduce the invasion and metastasis of colorectal cancer, and also indicate a good prognosis.However, integrin α5β1 plays an adverse role in colorectal carcinoma.E-cadherin and integrin α5β1 might play a synergistic role in the pathogenesis of colorectal carcinoma, and might become the clinical predictive markers in colon cancer.
