速度向量成像技术对评价急性动脉粥样硬化性脑梗死患者颈动脉斑块长轴应变及运动同步性的影响

         

摘要

目的 采用速度向量成像技术评价颈动脉斑块长轴方向的应变和运动同步性.方法 对急性动脉粥样硬化性脑梗死(ACI)患者50例和无脑血管病史(NCD)对照组40例进行颈动脉超声检查,采用速度向量成像测定斑块不同位点的最大应变(Smax)、应变率(SR)和应变达峰时间(Tp).结果 (1)ACI组检出偏心性斑块69个,直径狭窄率均小于50%,NCD组检出偏心性斑块45个,去除硬斑和溃疡斑,ACI组56个斑块,NCD组40个斑块纳入研究.(2)2组斑块近心端Smax大于顶部、顶部大于基底部;ACI组斑块近心端肩部Smax和顶部Smax小于NCD组;2组近心端肩部SR大于顶部,其余位点SR无统计学差异;2组间SR相比差异无统计学意义.(3)ACI组内近心端肩部Tp小于基底部Tp;2组间相比ACI组近心端肩部Tp小于NCD组.结论 速度向量成像可以定量检测斑块不同位点的应变和同步性,在一定程度上反映斑块易损性.%Objective To evaluate the strain and synchronism of plaque of carotid artery by using velocity vector imaging (VVI).Methods Fifty-nine patients with acute atherosclerotic cerebral infarction (ACI) and 40 non-cerebralvascular disorder (NCD) individuals underwent carotid uhrasonography.Velocity vector imaging was used to detect maximal strain (Smax),strain rate (SR) and time to the peak of strain (Tp) at different sites of the plaque.Results (1) Sixty-nine eccentricity plaques were detected in the ACI group and 45 plaques were detected in the NCD group.With the exclusion of stiff-plaque and ulcer-plaque,the 56 plaques in the ACI group and 40 plaques in the NCD group were included in the study.(2) The proximal Smax of the plaques at the shoulder in both groups was greater than that at the top,and the Smax at the top was greater than that at the base.The proximal Smax of the plaques at the shoulder and the Smax of the plaques at the top in the ACI group were all smaller than those of the NCD group.The proximal SR at shoulder in the 2 groups was greater than that at the top,but there were no differences in SR at other sites,and no statistical significance could be seen in SR,when comparisons were made between the 2 groups.(3) In the ACI group,the proximal Tp at the shoulder was smaller than that at the base.The proximal Tp at the shoulder in the ACI group was smaller than that at the base in the NCD group.Conclusion Velocity vector imaging could be applied to detect stain and synchronism at different sites of the plaques,and to a certain extent,it could be used to judge the vulnerability of plaques.
