分化型甲状腺癌二次手术安全性探讨

摘要

目的:探讨分化型甲状腺癌二次手术的安全性.方法:回顾分析2003年7月-2010年2月63例分化型甲状腺癌二次手术相关资料,对二次手术的并发症进行分析.结果:二次手术并发症发生率17.46%(11/63),暂时性甲状旁腺损伤发生率为7.94%(5/63),暂时喉返神经损伤发生率为4.76%(3/63),2周后声音嘶哑逐步恢复,1例发生永久性喉返神经损伤,先行呼吸机正压给氧待患者呼吸困难好转再行声带成形手术后患者呼吸困难消失,气管损伤发生率为3.17%(2/63).结论:分化型甲状腺癌二次手术因解剖组织结构改变,二次手术并发症发生率较高,进行二次手术应熟悉解剖、掌握术中操作技巧及谨慎操作.%Objective: To investigate the safety of reoperation of differentiated thyroid carcinoma. Methods: Data of 63 cases of reoperation for differentiated thyroid carcinoma was reviewed and the relative factors were analyzed. Results: The rate of complication of reoperation was 17.46％( 11/63 ). The rate of temporary hypocalcemia was 7.94％( 5/63 ). And the rates of temporary and permanent laryngeal recurrent nerve injury were 4.76％( 3/63 ) and 1.59％( 1/63 ), respectively. The injury rate of trachea was 3.17％ ( 2/63 ). Conclusion:Complication of reoperation of differentiated thyroid carcinoma is high and it should been carried out with familiarity in anatomy and attention.
