目的:比较左、右半结肠癌患者的临床病理特征及生存差异,分析左、右半结肠癌预后影响因素.方法:以总生存期作为观察指标,2010年1月至2015年12月398例手术切除的结肠癌患者作为研究对象,选用SPSS 16.0软件分析对比左、右半结肠癌之间的差异,探讨影响生存预后的可能因素.结果:Cox回归分析显示,肿瘤部位、大体分型、远处转移、TNM分期是结肠癌患者生存预后的预测因素.总中位生存期36.0个月.左半结肠癌组和右半结肠癌组中位生存期分别为39.0个月和32.0个月.与右半结肠癌相比,左半结肠癌的死亡风险降低了21.3％,P=0.032.左、右半结肠癌在分期、肿瘤最大径、肿瘤标志物、首发症状方面差异有统计学意义(P＜0.05),右半结肠癌较左半结肠癌病灶最大径大、分期晚、肿瘤标志物阳性率高.结论:左半结肠癌患者的生存预后优于右半结肠癌患者.肿瘤部位、大体分型和分期可能是结肠癌生存的预测因素.%Objective:To compare the clinicopathological features and survival differences of patients with left and right－sided colon cancer,and to analyze the prognostic factors of left and right－sided colon cancer in the First Affil-iated Hospital of Anhui Medical University.Methods:Taking OS as the observation index,from January 2010 to De-cember 2015,398 cases of surgical resection of colon cancer patients were selected as the research objects,with the SPSS 16.0 software to compare the differences between left and right－sided colon cancer,to explore the possible fac-tors affecting survival prognosis.Results:Cox regression analysis showed that tumor location,gross type,distant metas-tasis and TNM staging were predictors of survival prognosis of colon cancer patients.The median survival time was 36.0 months.The median survival time in the left and right－sided colon cancer group was 39.0 months and 32.0 months respectively.Compared with right－sided colon cancer,the risk of death from left－sided colon cancer was de-creased by 21.3％,P=0.032.The left and right－sided colon cancer had significant differences in staging,tumor di-ameter,tumor markers,initial symptoms(P＜0.05).Compared with the left－sided colon,right－sided colon cancer had larger lesions,later staging,higher tumor marker positive rate.Conclusion:The prognosis of patients with left－si-ded colon cancer is better than that of right－sided colon cancer patients.Tumor location,gross typing and staging may be the predictors of colon cancer survival and need further analysis.
