芪归胶囊联合甘露聚糖肽片治疗老年晚期直肠癌肝转移TACE术后患者的疗效及对免疫功能的影响

摘要

Objective:To investigate the clinical effects of the effects of qigui capsule combined with mannatide tablets in transcatheter arterial chemoembolization(TACE) therapy of hepatic rectal cancer metastases in elderly people.Methods:From January 2014 to January 2016,82 elderly patients with hepatic rectal cancer metastases treated in our hospital were randomly divided observation group (41 cases) and control group (41 cases).The patients were treated by TACE and mannatide tablets,while the observation group was given Qigui capsule.After treatment,the curative effects were evaluated in the two groups.The long-term effects were evaluated by observe the 1 year survival rate,2 year survival rate,OS,TFP in the two groups.The levels of CD3 +,CD4 +,CD8-and CD19 + in the two groups before and after the treatment were detected.Then the toxicity of the two groups was compared.Results:The total effective rate was 70.7％ in the observation group,higher than the 51.2％ in the control group(P ＜ 0.05).The 1 year survival rate of observation group was similar with the control group(P ＞ 0.05),while 2 year survival rate,OS and TTP were 58.5％,16.9 months and 11.3 months,significantly higher than which in the control group(P ＜ 0.05).The levels of CD3 +,CD4 +,CD8 + and CD19 + in the observation group were significantly higher than which in the control group(P ＜ 0.05).There were no serious toxicity occurred during the treatment in the two groups and the rates of toxicity were lower in the observation group than the control group(P ＜0.05).Conclusion:Qigui capsule combined with mannatide tablets in transcatheter arterial chemoembolization therapy of hepatic rectal cancer metastases in elderly people has a good effect and worth to the next study.%目的:探讨芪归胶囊联合甘露聚糖肽片对老年直肠癌肝转移行肝动脉化疗栓塞术(TACE)后患者的临床疗效.方法:将2014年1月至2016年1月于我院肿瘤科接受治疗的82例老年直肠癌肝转移患者随机分为观察组和对照组,每组41例.所有患者均给予TACE及全身化疗治疗,对照组另给予甘露聚糖肽片治疗,观察组在对照组治疗的基础上同时给予芪归胶囊进行治疗.治疗结束后评价两组近期疗效,对两组均进行为期2年的随访,观察患者1年及2年生存率、总生存时间(OS)和疾病进展时间(TTP);治疗前后分别抽取患者静脉血,检测其中CD3+、CD4+、CD8+及CD19+等T淋巴细胞亚群水平,观察两组患者毒副作用.结果:近期疗效评价中,观察组总有效率为70.7％,明显高于对照组的51.2％(P＜0.05);远期疗效评价中,两组1年生存率无明显差异(P＞0.05);观察组2年生存率、OS及TTP分别为58.5％、16.9个月及11.3个月,均显著高于对照组(P＜0.05).治疗后,观察组CD3+、CD4+、CD8+及CD19+水平均明显高于对照组(P＜0.05).治疗期间,所有患者均未出现严重药物毒副作用,且观察组各项毒副作用的发生率均显著低于对照组(P＜0.05).结论:芪归胶囊联合甘露聚糖肽片用于老年晚期直肠癌肝转移TACE术后治疗具有较好的疗效,值得进行深入研究.
