胸腔积液中AFU和Tch联合检测在良恶性疾病鉴别诊断中的价值

摘要

目的 探讨胸腔积液中α-L-岩藻糖苷酶(AFU)、胆固醇(Tch)在良恶性疾病鉴别诊断的价值.方法 用生化比色法检测临床上36例胸腔积液中AFU,Tch活性的变化.结果 恶性胸腔积液AFU及Tch分别为(172.55±90.38) μmol/(L·h)和(1.66±0.91)mmol/L;良性胸腔积液AFU及Tch分别为(102.01±56.52)μmol/(L·h)和(0.71±0.61)mmol/L.恶性胸腔积液组AFU活性明显高于良性胸腔积液组,差异有统计学意义(P＜0.01,t=3.534);恶性胸腔积液组Tch活性明显高于良性胸腔积液组,差异有统计学意义(P＜0.01,t=3.199).以AFU 119.5 μmol/(L·h),Tch 1.1 mmol/L为界值测定恶性胸腔积液的敏感度、特异度分别为67.23％,75.02％和气72.12％,86.19％.两项联合检测的敏感度、特异度为78.62％和90.02％.结论 胸腔积液中AFU和Tch的活性测定有助于恶性胸腔积液和非结核良性胸腔积液鉴别诊断,两者联合检测可提高良恶性胸腔积液鉴别诊断的敏感度和特异度.%Objective To study the value α-L-Fucosidase(AFU) and Tchactivity(Tch) in the differential diagnosis of pleural effusion. Conclusion Determination of AFU and Tch of pleural effusion is conductive to differential diagnosis of malignant and benign pleural effusion. Simultaneous determination of AFU and Tch can improve the sensitivity and specificity in the differential diagnosis of pleural effusion.
