NF-κB p50亚基在原发结内的非特指型外周T细胞淋巴瘤中的表达

摘要

目的 探讨原发结内的非特指型外周T细胞淋巴瘤(PTCL-U)核转录因子(NF-κB p50)的表达与其复杂的生物学行为的关系.方法 应用免疫组织化学方法,检测51例经病理确诊的原发结内PTCL-U患者病理组织的NF-κ Bp50、p170糖蛋白的表达情况,分析其与临床特征、治疗效果、预后的关系.结果 51例患者中NF-κB p50和p170糖蛋白的阳性表达率分别为21.6%(11/51)和60.8%(31/51).NF-κBp50和p170糖蛋白的表达和一般行为状态差(PS＞2分)及首程治疗不能完伞缓解显著相关(Spearman相关系数=0.459、0.313、0.284;P=0.001、0.025、0.044).NF-κ B p50表达阳性组总的生存率经Log-Rank检验显著低于阴性组(P=0.0451).多因素生存分析显示:一般行为状态差(PS＞2分)、Ki-67高表达为PTCI-U的独立预后不良冈素,NF-κ B p50尚不能作为独立预后因素.结论 NF-κ B p50的表达和原发于结内的PTCL-U的耐药性和不良预后有关.%Objective To study the expression of NF-κB p50 in nodal peripheral T-cell lymphomasunspecified (PTCL-U),and investigated the relationship between NF-κB and PTCL-U's complex biological behavior. Methods 51 patients with nodal PTCL-U were analysed by detecting the expression of NF-κB p50, p170 by immunohistochemistry and correlation between them and PTCL-U' s clinical feature, treatment effectiveness and prognosis were also studied. Results 11 patients(21.6 %, 11/51) and 31patients (60.8 %,31/51) were respectively positive for N F-κB p50 and p 170 expression. Expression of NF-κB were significantly correlated with p170 expression, poor performance status (PS＞2) and non-complete remission in first line treatment(Spearman correlation= 0.459, 0.313, 0.284; P = 0.001, 0.025, 0.044). Overall survival rate of NFκB p50-positive PTCL-U was significantly lower than that of NF-κB p50-negative patients by Log-Rank test (P =0.0451). Multivariate analysis showed poor performance and higher Ki-67 were independent prognostic factor for PTCL-U, while NF-κB p50 was not. Conclusion The expression of NF-κB pS0 was correlated with muhidrug resistance and poor prognosis in nodal PTCL-U.
