基于正交设计的不同针刺因素对皮质酮肾阳虚大鼠垂体-性腺轴的影响

         

摘要

Objective:To explore primary&secondary factors that influence acupuncture effectiveness and the best combination of acu-puncture parameters, in order to provide experimental evidence to elevate acupuncture effectiveness for clinic application.Orthogonal design was adopted to study several factors that influence acupuncture effectiveness.The rats model of kidney-YANG deficiency made by hydrocortisone as the study objects.Methods:4 influential factors ( i.e.selection of acupoints, twisting-rotating angle, twisting-rotating frequency and acupuncture time) plus 3 levels of each factor were arranged to L9 (34 ) orthogonal table.Treat kidney-YANG deficiency rats by computer controlled acupuncture manipulation device.Serum T and E2 were evaluated by ELISA method and pituita-ry LH and FSH mRNA expression by real time fluorescent quantitation PCR to illustrate the primary&secondary factors and the good&bad of different levels in each factor that could affect the acupuncture effectiveness.Results:Among 4 factors mentioned above, the order for influential factors from major to small is acupoints selection, twisting-rotating angle, twisting-rotating frequency and acu-puncure time, among which, selection of acupoints was the major factors ( P<0.05) , twisting-rotating angle, twisting-rotating fre-quency and acupuncture time were the secondary factors (P>0.05,P>0.05 and P>0.05) .Conclusion : ①The best acupuncture parameters for treating kidney-YANG deficiency rats were:selecting Shenshu acupoint (B23), 90°of twisting-rotating angle, 60tpm of twisting-rotating frequency and 60s of acupuncture time;②In this study, different acupuncture parameter has different efficacy for kidney-YANG deficiency rats;among them, selection of acupoints became the biggest influential factor, the next was twisting-rota-ting angle, the secondary was twisting-rotating frequency, and the last one was acupuncture time.%目的:探索影响针刺效应的主次因素和针刺参数的最佳组合,以期为提高针刺疗效、指导针灸临床提供理论依据。方法:采用L9(34)正交表对影响针刺效应的4因素(穴位的选择、捻针角度、捻针频率、捻针时间)3水平安排试验,运用计算机控制针刺手法仪治疗肾阳虚大鼠,通过酶联免疫检测血清T、E2和实时荧光定量PCR检测垂体中LH、FSH mRNA表达的水平来比较上述4因素的主次。结果:上述4因素对针刺效应的影响大小依次是穴位选择、捻针角度、捻针频率、捻针时间,其中腧穴的选择是影响针刺效应的重要因素(P<0.05);捻针角度、捻针频率和捻针时间是影响针刺效应的次要因素(P>0.05)。结论:针刺治疗肾阳虚大鼠的最佳针刺参数为针刺肾俞穴,捻针角度为90°,频率为60次/min,捻针60s;不同因素对针刺治疗肾阳虚大鼠的疗效不同,其中穴位的选择对疗效的影响最大,其次为捻针角度,再次是捻针频率,最次是捻针时间。
