目的 观察甲硝唑纱条联合挂线术对促进肛周脓肿创面愈合的效果.方法 纳入本院80 例肛周脓肿患者作为研究对象,随机分为两组,每组各40 例.观察组采用甲硝唑纱条联合挂线术,对照组采用高锰酸钾坐浴、凡士林纱条联合挂线术.比较两组手术疗效及创面愈合质量.结果 观察组痊愈33 例,对照组痊愈30 例,两组痊愈率和总体手术疗效差异均无统计学意义(均P＞ 0.05),观察组术后3 d 和1 w 时VAS 评分低于对照组(均P ＜ 0.05),观察组术后3 d 和1 w 时创面渗出评分高于对照组(均P＜ 0.05).两组肉芽组织生长评分差异均无统计学意义(均P ＞ 0.05).观察组创面腐肉脱落时间和创面愈合时间短于对照组(均P＜ 0.05),观察组术后3 w 时创面愈合率高于对照组(P ＜ 0.05).结论 甲硝唑纱条联合挂线术用于肛周脓肿能缩短创面愈合时间,促进患者早期康复.%Objective To study the effect of metronidazole gauze combined with seton on incision and wound healing of perianal abscess. Methods 80 cases of perianal abscess were included in the study. They were divided into two groups by random draw, with 40 cases in each group. The treatment group received seton combined with metronidazole gauze, and the control group received seton, potassium permanganate sitz bath and vaseline gauze. The curative effect and the quality of incision and wound healing were compared between groups. Results 33 cases were cured in the treatment group and 30 cases were cured in the control group. There were no significant difference in the cure rate and the overall operation effectiveness between the two groups (P ＞ 0.05). The VAS scores at 3 days and 1 week after the operation were lower in the treatment group than in the control group (P ＜ 0.05). The wound exudation scores at 3 days and 1 week after the operation were higher in the treatment group than in the control group (P ＜ 0.05). There was no significant difference in the growth score of granulation tissue between the two groups (P ＜ 0.05). The carrion shedding time and wound healing time in the treatment group were shorter than in the control group (P ＜ 0.05). The healing rate in the treatment group at3 weeks after the operation was higher than that in the control group (P ＜ 0.05). Conclusion Seton combined with metronidazole gauze can shorten the healing times of incision and wound. It can promote the early rehabilitation of the patients with perianal abscess.
