射频消融治疗对肝癌患者细胞免疫功能的影响△

摘要

目的 探讨射频消融治疗对肝癌患者细胞免疫功能的影响.方法 对60例肝癌患者应用射频消融术治疗,于治疗前24 h 、治疗1周及4周末采用流式细胞仪检测患者外周血 T 淋巴细胞亚群及CD4+CD25+调节性T细胞的比例变化,治疗4周末依据临床症状缓解情况评定临床疗效.结果 治疗4周末,本组患者治疗总有效率为50%.射频消融治疗1周、4周末患者外周血中CD3+、CD4+及CD4+ ／CD8+水平均较治疗前升高,CD4+CD25+调节性T细胞较治疗前降低,差异有统计学意义(P＜0 .05) ;CD8+水平略升高,差异无统计学意义(P＞0 .05).与治疗1周末比较,治疗4周末CD3+、CD4+及CD4+ ／CD8+比例略有升高,CD4+CD25+调节性T细胞比例略升高,差异无统计学意义(P＞0 .05) .结论 原发性肝癌患者存在细胞免疫抑制,射频消融治疗能明显提高患者的细胞免疫功能.%Objective To investigate the effect of radiofrequency ablation on cellular immune function in patients with liver cancer .Methods 60 patients with liver cancer were treated by radiofrequency ablation . Flow cy to metry w as used to detect the proportion of peripheral blood T ly mphocy te subsets and CD 4+CD25+ regulatory T cells at 24 h before treatment ,1 week and 4 weeks after treatment .At the end of the 4th w eek of treatment ,the clinical efficacy w as evaluated according to the clinical sy mptom relief .Results A t the end of the 4th w eek of treatment ,the total effective rate of treatment in this group of patients w as 50% .A t the end of the first ,4th w eek of treatment ,the levels of CD3+,CD4+ and CD4+ ／CD8+ in peripheral blood of patients were higher than those before treatment ,and CD4+ CD25+ regulatory T cells w ere low er than before treatment , the difference w as statistically significant ( P＜ 0.05).T he level of CD8+ w as slightly elevated ,and the difference w as no t statistically significant ( P＞0 .05).Compared w ith the first w eek of treatment ,the proportion of CD3+,CD4+ and CD4+ ／CD8+ increased slightly at the end of the 4th w eek ,and the proportion of CD4+ CD25+ regulatory T cells increased slightly , the difference w as not statistically significant ( P＞ 0 .05 ).Conclusion Cellular im munosuppression exists in patients w ith primary liver cancer ,and radiofrequency ablation can significantly improve the cellular im m une func- tion of patients .
