目的 探讨负压封闭引流术联合生肌散治疗肛周坏死性筋膜炎的临床疗效及对外周血炎症因子表达的影响.方法 将68例肛周坏死性筋膜炎患者采用随机数字表法分为观察组与对照组,各34例.两组均给予负压封闭引流术治疗,观察组联合生肌散外敷治疗.比较治疗前后两组患者外周血炎症因子水平,比较两组创面愈合时间、住院时间、有效率及复发率.结果 治疗后两组炎症因子水平均显著下降(P＜0.05),观察组较对照组下降更显著(P＜0.05).观察组创面愈合时间及住院时间均显著少于对照组(P＜0.01),总有效率显著高于对照组(P＜0.05),复发率显著低于对照组(P＜0.05).结论 负压封闭引流术联合生肌散治疗肛周坏死性筋膜炎效果显著,可有效抑制炎症反应,促进创面愈合,值得进一步推广应用.%Objective To investigate the curative effect of vacuum sealing drainage combined with Shengjisan in the treatment of perianal necrotizing fasciitis and its influence on inflammatory cytokine expression in peripheral blood. Method 68 patients with perianal necrotizing fasciitis were randomly divided into the observation group and the control group, 34 cases in each group. Patients in the control group were treated with vacuum sealing drainage. Patients in the observation group were treated with vacuum sealing drainage combined with Shengjisan external therapy. Inflammatory cytokine levels in peripheral blood of the two groups before and after treatment were compared. Wound healing time, hospitalization time, effective rate and recurrence rate of the two groups were compared. Results After treatment, inflammatory cytokine levels in the two groups were significantly declined (P ＜ 0.05), the observation group declined more significantly than the control group (P＜0.05). Wound healing time,hospitalization time in the observation group were significantly shorter than those in the control group (P＜0.01),total effective rate was significantly higher than that of the control group (P＜0.05), the recurrence rate was significantly lower than that of the control group (P＜0.05). Conclusion Vacuum sealing drainage combined with Shengjisan in the treatment of perianal necrotizing fasciitis have significant curative effect, it can effectively inhibit inflammatory response and promote wound healing, it is worthy of further popularization and application.
