颅脑超声与血清NSE对早产儿脑白质损伤诊断价值及二者相关性分析

         

摘要

目的 探讨颅脑超声灰度值以及血清神经元特异性烯醇化酶(NSE)对脑白质损伤早产儿早期诊断和预后评估的价值.方法观察组为脑白质损伤早产儿35例,对照组为正常早产儿36名,分别于生后第1、7、14 d检测NSE水平,并进行颅脑超声检查.结果观察组早产儿出生第1天颅脑超声异常率为97.2%,第7天为86.1%,第14天为83.3%,均显著高于对照组(P<0.01);观察组早产儿出生后第1、7及14天,血清NSE值显著高于对照组(P<0.01);观察组早产儿出生后第1、7及14天,颅脑超声灰度值显著高于对照组(P<0.01);颅脑超声与NSE呈正相关(r=0.826,P=0.000).结论颅脑超声和NSE可作为早产儿脑白质损伤早期诊断和预后评估的有力依据.%Objective To analyze the ultrasound gray scale and neuron specific enolase (NSE), and to determine their value in early diagnosis and prognosis of white matter damage in premature infant. Methods Thirty-five preterm infants with while matter damage were allocated to the observation group and 36 healthy preterm infants were selected as the control group. NSE levels were measured in these two groups by ELTSA. Ultrasound examination was conducted on the j st day, the 7th day arid the 14 th day after birth. Results The rates of ultrasound abnormalities in the observation group was 97.2% on the 1st day, 86.1% on the 7th day and 83.3% on the 14th day after birth. The rates in the observation group were much higher than those in the control group (P<0.01). NSE in the observation group was significantly higher than that in control group on the 1st day, the 7th day and the 4th day after birth (P<0.01). Ultrasound gray scale in the observation group was significantly higher than that in the control group on the 1st; day, the 7th day and the 14th day after birth (P<0.01). There was a positive correlation between NSE and ultrasound gray scale (r= 0.826, P=O.000). Conclusions Ultrasound and NSE could be used in early diagnosis and prognosis of white matter damage in preterm infants.
