Objective To explore the surgical techniques and early results of Metha short stem implantation combined with OCM( Orthopaedische Chirurgie Miienchen ) approach in total hip arthroplasty. Methods 12 cases of end-stage hip diseases and injuries ( 13 hips ) received total hip arthroplasties with surgical techniques of Metha short stem implantation and OCM approach. They were allowed to leave the bed with the assistance of a walker on the third day after surgery and to take full weight bearing without any assistance 1 month after surgery. Results All the cases were followed up for 10 ~ 24 months. All the patients achieved significant improvement of hip function. Harris scores went up to 88 ~ 96 points postoperatively. No complication such as prosthesis loosening, hip dislocation, deep infection and thrombosis embolism happened. Conclusions Metha short stem has advantages of bone stock preservation owing to its small size and metaphyseal fixation mechanism. If it performed with proper techniques of implantation through the OCM approach, it can achieve good results.%目的 探讨OCM入路下Metha短柄假体植入的手术技术和早期效果.方法 对12例髋关节晚期伤病患者(13髋)采用髋关节OCM入路下Metha短柄全髋关节置换术.术后第3天在助行器帮助下下地,术后1个月无需任何辅助完全负重行走.结果 12例手术全部成功.患者均获随访,时间10~24个月.髋关节功能均明显改善,按Harris 评分为88~96 分.无假体松动、关节脱位、假体深部感染和深静脉血栓栓塞等并发症发生.结论 Metha短柄具有外形小巧、依靠干骺部固定的优势,同时也保存了股骨近端的骨量.配合OCM入路,注意正确的植入技术,可以取得良好的早期临床效果.
